Families are required to submit their letter indicating they qualify for free or reduced lunch at least once every two years. You can upload that letter here, email, or mail it to us. If you need to request the new letter, please contact CPS Nutrition Services at (573) 214-3480 or lunch@cpsk12.org.

Please list any other items the child needs in order to participate in the activity that you would like to request the Day Dreams Foundation to provide funds for (ex: shoes, uniform, equipment). If you know the cost of the items (or if there is a specific place these must be purchased) please list those as well. Please note that our $500 maximum scholarship per child, per calendar year includes equipment costs as well.
